How to find a primary care physician or nurse practitioner
If you are looking for a new family physician, we suggest contacting Health Care Connect to register for a family health care provider in your area who may be accepting new patients. Health Care Connect is an initiative by the Ministry of Health and Long-Term Care to assist Ontarians who are seeking primary health care providers. For more information about Health Care Connect, please visit the Ministry’s website, https://hcc3.hcc.moh.gov.on.ca/, or call 1-800-445-1822.
You can use a nurse practitioner as your primary care provider in one of the NP-led clinics in Ontario. Some of these clinics may offer telemedicine services. There are more than a dozen of them listed here: https://www.allianceon.org/nurse-practitioner-led-clinics.
