About the Service

We provide one-time psychiatric assessments with the option for follow-up consultations as needed. Ongoing psychiatric care is not offered.
Urgent appointments are available and most visits are scheduled within 2 to 6 weeks.

Referral Process

Who can refer: Physicians and Nurse Practitioners licensed in Ontario.

How to refer:
Please send a referral letter to:

OR

• FAX: 905-225-6523

Include the following details in your referral:

• Patient's full name

• Age

• Valid Ontario health card number

• Parent/Representative's phone number and email

• Referring provider’s billing number

After Referral

Once the referral is received:

• The family will receive an email with a secure Zoom for Healthcare link to schedule the appointment.

• A comprehensive consultation report will be sent to the referring provider within 24 hours of the appointment.
